What you need to know

If you are attending our clinic for the first time, please download and complete our New Patient Registration Form. You can bring your forms with you on the day of your consultation, or fax or email the forms prior to your appointment. This will reduced the amount of waiting time before your appointment. 

Please arrive 15 minutes before your appointment time to complete your New Patient Registration Form to avoid delays. Please also bring:

* For Worker’s Compensation patients only

Without these details you will be required to pay for the consultation on the day and take responsibility for claiming back payment from your employer or insurance company.

Macquarie University Clinic
There is ample parking available under the hospital building (which is cheaper than the on street parking and the parking in front of the clinic building itself). If you park under the hospital, take the elevator to the ground floor and walk across Technology Place to the Clinic Building. We are on Level 3 in Suite 303.

Lakeview Private Hospital
There is ample parking on the hospital grounds. The entry into the hospital is off Solent Circuit, however, your need to be travelling south on Solent Circuit due to a median strip.

We accept cash, EFTPOS and most major credit cards (Visa, Mastercard and American Express). For surgery payments. Direct Deposit into our account can be organised and is appreciated.

Please bring:

 

  • Your referral
  • Your x-rays, CT scans and MRI scans
  • Your Medicare and Private Health Details
  • New Patient Registration Form or arrive 15 mins early to fill in on the day
  • Details of your employer, address, phone number and contact person at your employment *
  • Workers’ compensation insurance approval letter, case manager, phone number and worker’s compensation claim number. *
